Individual assembly names are listed only when there is more than one in a particular city, otherwise only the city name appears.  The date of commendation shown is the year you left for the field with your current commendation.  If you have a commendation by an assembly other than the one listed, we must have a copy of that letter of commendation to include it in the 2008 Handbook.
